One phone shows a code, the other scans it. Then a few verification words appear on both phones: you read them out loud and check that they match. If they do, the link is the right one and nobody else can have slipped in between.

Diary events, the photos you attach, growth measurements, reminders and the medical card stay aligned. The child's profile reaches the other phones too: after linking, each phone keeps the name and emoji it prefers, and the profile photo is chosen by the phone that manages the child. Everything is encrypted on the phone before it leaves, and the keys stay on the linked phones.
The list of phones, under your control
In the settings you find the list of phones linked to that child, each one with its name. From there you can break the link whenever you want, without asking anyone's permission.
